Create a check list that is easily located and up to date. This eliminates additional stressors during medical emergencies/appointments allowing for more time to focus on important questions for providers rather than searching for information.

Keep the information up-to-date over time and as information changes.
Information to include:
Your medical history
Medication lists and any allergies
Your insurance information
The names and phone number of all of your doctors
Think about putting the files together in a bright colored folder and place it in a prominent place such as on your refrigerator.
